Job Title: Pediatric Social Worker
Job Type: Full-Time (Hybrid – Onsite visit once in a week)
Location: Bronx, NY
Summary:
We are seeking a compassionate Licensed Master Social Worker (LMSW) to provide care and support to children and families. This is a Hybrid position. Onsite visit once in a week.
Job Description:
- Provide counseling and support services to children ages 6 and above and their families
- Conduct assessments and develop individualized care plans
- Collaborate with healthcare professionals and community resources
- Ensure compliance with Medicaid and Medicare guidelines
- Maintain accurate documentation and case records
Requirements:
- Minimum 2 years of experience in social work, preferably with children
- Active New York State social worker license
- Must have experience with Medicaid and Medicare
- Ability to work with children ages 6 and above
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
