For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.

Caring Communities provides housing and mental health services to those with documented history of homelessness, substance abuse, and chronic mental illness.

STATEMENT OF THE JOB:

The Social Work Supervisor acts as a role model in their professional behavior for the staff and members of the Residence. The Social Work Supervisor supervises the Housing Wellness Coordinators job performance of organized program activities, group discussions, individual conversations and case documentation. The Social Work Supervisor responds directly to the Clinical Director and is responsible for the growth and maintenance of the Program.
* Supervises staff at Caring Communities
* Maintain a safe and peaceful environment including managing client’s conflicts, symptomology, and difficulties while ensuring the rights, dignity and safety of all clients.
* Provide crisis intervention and advocacy services.
* Carries a small caseload.
* Provides leadership to Housing Wellness Coordinators, including reviewing and signing off on all chart work on a timely basis, meeting individually with staff for supervision and providing group supervision.
* Facilitate coverage for staff in all units while encouraging member participation in unit activities daily.
* Facilitate various groups.
* Complete performance appraisals for staff.
* Aid staff in the evaluation of members as well as the appropriate formulation of treatment plans.
* Assist in the intake process for the program.
* Adhere to all documentation requirements in charts. This includes but is not limited to the appropriate completion of Inquiry Log Sheets, Client Profile forms and other agency and government surveys, and Group Logs documenting client participation.
* Update information in charts to maintain audit standard requirements.
* Arrange intakes and ensure complete and appropriate intake documentation on all new members.
* Ensure that referrals are made to appropriate internal and external services as required to meet the consumer’s or families needs.
* Follow-up on referrals you make on behalf of a consumer/family and respond to all referrals made to you / your program.
* Provide coverage when staff is unavailable during all hours of operation including some holidays.
* Participate in cross-program service reviews that support the planning and coordination of service delivery closure.
* Participate in agency’s mental health, and program special events, activities, projects, workshop, and training.
* Arrange for and accompany members on trips, job interviews and outings off-premises.


QUALIFICATIONS:

* MSW preferred; MA/MS in Human Services or related field required.
* Certified Rehabilitation Counselor required; or NYS LMSW License preferred.
* Prefer two years’ experience in mental health. field
* Experience in supervision and with person diagnosed with psychiatric disabilities population preferred.
* Proficient in computer software applications
* Ability to work a flexible schedule providing evening, weekend, and holiday coverage as needed
* Able to talk, hear, walk, stand, and sit.
* Able to lift up to 10 pounds.
