The Adult Homes Initiative is designed to provide peer support services to people transitioning from adult homes into the community through individual and group peer support meetings. <li>Support clients in accessing mental health services, medical care, substance treatment, vital documents, benefits, legal/immigration services, and permanent housing</li>
<li>Document all interactions with or on behalf of clients; monitor and document client progress toward service plan goals</li>
<li>Maintain familiarity with treatment, rehabilitation, and community support programs available in the community</li>
<li>Coordinate client care with other service providers; attend regular case conferences with clients and other service providers, as required</li>

QUALIFICATIONS 
<ul>
<li>Master’s degree or higher in social work, mental health counseling, nursing, or psychology; relevant licensure is required within 6 months of hire; Credentialed Alcoholism and Substance Abuse Counselors also welcome to apply</li>
<li>Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points</li>
<li>Prior experience working with homeless and/or precariously housed populations preferred, but not required</li>
<li>Ability to engage and build rapport with populations with high support needs</li>
<li>Knowledge of counseling principles and methods for mental illness and substance use disorders</li>
<li>Knowledge of techniques for identifying, assessing, and preventing potentially violent behavior, including crisis management and de-escalation techniques</li>
<li>Strong oral and written communication skills</li>
<li>Detail-oriented and resourceful with strong time management and organizational skills</li>
<li>Proficiency in Microsoft Office suite and other standard business technology</li>
<li>Bilingual/multilingual preferred, but not required</li>

We are currently seeking a dedicated RN Nurse Manager within our Adult and Pediatrics Nursing home health care team to lead our clinical team.
Position Summary:
The RN Nurse Manager oversees the day-to-day clinical operations of pediatric home care services. This role ensures that all care provided meets clinical standards, regulatory requirements, and organizational goals. The RN Nurse Manager will be responsible for supervising nursing staff, managing care plans, collaborating with interdisciplinary teams, and maintaining strong relationships with patients and their families.
Key Responsibilities:
<ul>
<li>Lead and supervise a team of registered nurses (RNs), licensed practical nurses (LPNs), and other field staff, ensuring high-quality and compassionate care is consistently delivered</li>
<li>Manage day-to-day clinical operations, including patient assignments, and case load oversight</li>
<li>Perform initial patient assessments, including home safety evaluations, and develop individualized plans of care in collaboration with physicians and families</li>
<li>Conduct supervisory home visits to ensure adherence to care plans, nursing standards, and regulatory guidelines</li>
<li>Monitor clinical documentation for accuracy, timeliness, and compliance with state, federal, and accreditation standards (e.g., CMS)</li>
<li>Participate in performance evaluations, corrective actions, and staff development plans for nursing personnel</li>
<li>Provide clinical guidance and support to field staff for complex cases or emergency situations</li>
<li>Collaborate with intake, scheduling, and administrative departments to support smooth patient transitions and service coordination</li>
<li>Lead and/or participate in interdisciplinary case conferences and care team meetings</li>
<li>Serve as a clinical liaison with physicians, specialists, schools, therapists, and external case managers</li>
<li>Ensure proper implementation of infection control, medication safety, and emergency protocols in the home setting</li>
<li>Assist in the recruitment, interviewing, onboarding, and orientation of new clinical staff</li>
<li>Maintain current knowledge of pediatric conditions, treatments, and home care trends through continuing education</li>
</ul>
Qualifications:
<ul>
<li>Registered Nurse (RN) with a valid state license (required)</li>
<li>Minimum of 2 years nursing experience</li>
<li>Prior experience in clinical supervision or nurse management a plus but not required!</li>
<li>Strong leadership, communication, and organizational skills</li>
<li>Ability to travel locally for home visits and supervisory duties</li>
<li>Trach / vent experience or an interest in learning!</li>
