Mental Health Association of Nassau County (MHANC) is a not-for-profit membership organization dedicated to improving mental health in our community through advocacy, education, program development, and the delivery of direct services. Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming a Care Coordinator.
 
The Care Coordinator functions as a member of an interdisciplinary team to provide care coordination to a caseload of severely mentally ill adults with multiple medical and/or co-occurring substance use disorders and/or medically ill individuals. Advocates for and supports the client, engages with community agencies/health care providers and others on his or her behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/or hospitalizations. Provides support to the team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions and resources to maintain focus on outcomes and best practices.
 
Essential Duties &amp; Responsibilities
<ul>
<li>Conducts initial and ongoing assessments of assigned clients to document strengths, needs, goals and resources.</li>
<li>Participates in the development/documentation/review and update of client centered comprehensive, integrated, interdisciplinary care plan in consultation with the Care Manager and other team members to ensure focus on desired outcomes.</li>
<li>Maintains effective communications with clients, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources and other agency staff on behalf of clients.</li>
<li>Maintains documents, records, statistics, and other related reports in an organized, timely and accurate manner.</li>
<li>Coordinates care planning with other providers of services/resources to ensure goal directed, collaborative care, including care transitions.</li>
<li>Works as a part of a Care Coordination team; attends and participates in team meetings to provide input/feedback around psychosocial conditions to review client status, update plans and goals and review outcomes to further program goals.</li>
<li>Acts as a resource/consultant to all team members on psychosocial and or substance abuse issues and resources.</li>
<li>Provides telephonic as well as face to face outreach, engagement and service planning in the field.</li>
<li>Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.</li>

Requirements:
 
Education &amp; Experience
<ul>
<li>High School diploma/GED with additional training and experience in human service delivery required, plus one (1) year of related human services experience required in providing direct services to mentally ill clients, in order to link them to a broad range of services essential to successfully living in the community.</li>
<li>Associate’s Degree with additional training and some experience in human service delivery preferred. </li>
<li>Bachelor’s Degree in the human service field. (Degrees in other related areas will be considered)</li>
</ul>
Other Qualifications 
<ul>
<li>Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model.</li>
<li>Knowledge of Medicaid, Social Security and other entitlements preferred.</li>
<li>Excellent interpersonal skills required.</li>
<li>Must have the ability and willingness to regularly travel to various locations in Nassau County, using agency vehicle when transporting consumers.</li>
<li>NYS Driver’s License required; must take defensive driving course through agency or external provider</li>

Job Summary:
The Service Coordinator will work with tenants, who are all seniors, at the Woodstock Hotel. The program provides case management services, information, referrals, counseling, housing assistance, and crisis intervention for clients and their families. The Service Coordinator is responsible for conducting detailed assessments to determine client needs and eligibility for services, engaging with clients to develop and implement a service plan and providing supportive case management and counseling on a regular, on-going basis. 
Responsibilities:
<ul>
<li>Conduct intake and assessment screening, assessing mental and physical health needs and supports, and financial eligibility for benefits, entitlements, conduct home and office and community visits to address unmet needs.</li>
<li>Implement constructive short and long-term interventions through counseling, education about counseling supports, case management, advocacy, referral for corollary services, long-term planning, and assistance with benefits.</li>
<li>In collaboration with Property Management assist with income affidavits,rental subsidy maintenance, housing applications and recertifications and assist in collecting rentas needed. </li>
<li>Provide direct support and education to caregivers (clients and families) on topics that can help support them in their caregiving capacity (e.g., self-care, health and wellness, legal planning, assisting care receivers with daily needs).</li>
<li>Provide timely documentation and maintenance of client files and progress notes.</li>
<li>Provide coverage for non-assigned program participants, as needed.</li>
<li>Assist with training (after six months to a year) new staff on essential job functions.</li>
<li>Perform other related tasks as requested or assigned.</li>
</ul>
Qualifications:

<li>Bachelor's degree in Social Work or Gerontology required. Licensed LMSW or LCSW preferred with at least 2 years relevant experience in social services; assistance with benefits a plus.</li>

<li>Bilingual language Spanish fluency preferred.</li>

Detailed Responsibilities:

<li>Functions as the main contact and support for their respective staff, patients, physicians, providers, and all other parties</li>
