Summary:
Case Managers provide comprehensive services to individual clients and families residing in cluster
apartments. Services provided by Case Managers include assessment, case planning, developing an
Independent Living Plan, case facilitation and management, counseling, referrals, information
workshops and support groups. The Case Managers coordinates with other program staff and local
agencies to provide short and long term care necessary to meet the needs related to moving families
to permanent housing.
Essential Job Functions:
The Case Manager is responsible for managing a caseload of families and children residing in the
shelter. Provides counseling intervention services to residents and coordinates all client services as
identified in the Independent Living Plan (ILP).
Major Duties and Responsibilities:

<li>Intake and assessment for families referred to the shelter.</li>
<li>Manage a caseload of families referred to the shelter by DHS.</li>
<li>Conduct homes visits at least twice a week to meet and discuss resident’s Independent Living Plans.</li>
<li>Conduct bi-weekly progress meetings with residents to assess progress.</li>
<li>Provide crisis intervention and short-term individual counseling.</li>
<li>Providing information and counseling to families on services and planning to move into permanent housing.</li>
<li>Advocate on behalf of clients for public assistance and entitlements with city agencies.</li>
<li>Case management, preparation of case records, prepare monthly and quarterly statistical reports.</li>
<li>Mediation for internal conflicts in the living quarters.</li>
<li>Group facilitation and conducting of educational presentations for communities.</li>
<li>Provide updates as appropriate and assist clients in reporting any challenges to moving to permanent housing.</li>
<li>Serve as an advocate and liaison between the family and various agencies, such as HRA and ACS.</li>
<li>Maintain routine administrative responsibilities to include the preparation of correspondence, maintaining, files, mail, email, and telephone.</li>
<li>Perform related tasks as requested.</li>

Knowledge and Skills:

Education:

<li>Master's degree in Social Work preferred or related field and a minimum of one year of case management</li>
<li>experience; or Bachelors Degree and a minimum of two years of case management experience.</li>
<li>Experience with homeless families and knowledge of issues of homelessness and/or residential services</li>
<li>preferred.</li>
<li>Strong interpersonal skills required.</li>
<li>Computer proficiency required including, but not limited to Excel, PowerPoint and Publisher.</li>
<li>Bilingual (English/Spanish) preferred.</li>

Upon clicking "Submit Application", you will be directed to this 2nd step"* Job Summary The Member Services Representative will be responsible for creating a positive member experience by providing a superior level of customer service to Planet Fitness members, prospective members and guests. Essential Duties and Responsibilities 
<ul>
<li>Greet members, prospective members and guests, providing exceptional customer service.</li>
<li>Respond to member questions and concerns in a timely and professional manner and elevate to Assistant Manager or Manager as needed.</li>
<li>Handle all front desk related activities including:
<ul>
<li>Answer phones in a friendly manner and assist callers with a variety of questions.</li>
<li>Check members into the system.</li>
<li>New member sign-up.</li>
<li>Take prospective members on tours.</li>
<li>Facilitate needed updates to members accounts.</li>
</ul>
</li>
<li>Assist in maintaining the neatness and cleanliness of the club.
<ul>
<li>Cleaning and sanitizing of all exercise equipment and Black Card spa amenities.</li>
<li>Ensuring restrooms are clean and sanitized, stocked and clutter free by following the routine cleaning schedule.</li>
<li>Perform a comprehensive walk of all club areas; quickly fix any safety/policy issues and address cleanliness concerns.</li>

Qualifications/Requirements 
<ul>
<li>Customer service background preferred.</li>
<li>Must be 18 years of age or older.</li>
<li>Basic computer proficiency.</li>
<li>A passion for fitness and health.</li>
<li>Upbeat and positive attitude!</li>
<li>Punctuality and reliability is a must.</li>
<li>Exceptional customer service skills; able to interact in a positive and professional way with members and co-workers, exceeding the members expectations.</li>
<li>Strong listener with the ability to empathize and problem solve.</li>
<li>Demonstrate diplomacy in all interactions while using appropriate behavior and language.</li>
<li>High School diploma/GED equivalent preferred.</li>
</ul>
Physical Demands 
<ul>
<li>Continual standing and walking during shift.</li>
<li>Continual talking in person or on the phone during shift.</li>
<li>Must be able to occasionally lift up to 50 lbs.</li>
<li>Will encounter toxic chemicals during shift.</li>
<li>Moving self in different positions, including bending, climbing, balancing, kneeling, crouching, and twisting, to accomplish tasks.</li>
