SUMMARY OF POSITION: Identifies individuals and/or members of their families within the target population who are in need of preventive and primary health care services and links them to the services they need. Includes care for: acute; chronic and communicable diseases; dental services; prenatal care and family planning; WIC and other nutrition services; pediatric care and immunizations.
