Required:
<ul>
<li>At least 6 months of HHA experience</li>
<li>Valid NYS HHA Certificate</li>
<li>Valid driver’s license</li>
<li>Pre-employment physical exam done within the past 12 months</li>
<li>PPD/QuantiFERON within the past 12 months or chest x-ray within the past 5 years</li>
<li>MMR Titers (proof of vaccination not accepted)</li>
