Responsibilities
- Assist case managers in organizing and managing client files
- Schedule and coordinate client appointments
- Conduct preliminary assessments and client interviews
- Aid clients in completing forms and documentation
- Provide clients with service and resource information
- Conduct follow-ups to confirm clients are receiving needed support
- Maintain and update accurate client records
- Prepare and distribute reports and case summaries
- Support community outreach initiatives and events
- Collaborate with team members to deliver comprehensive services
- Provide transportation for clients to appointments, if needed
Qualifications
- Prior experience in social services or a similar support role preferred
- High school diploma or equivalent (required)
- Strong organizational and time management skills
- Excellent written and verbal communication
- Proficiency with basic computer applications and case management software
- Ability to interact with empathy, discretion, and professionalism
- Experience working with diverse populations
- Bilingual/multilingual candidates are strongly encouraged to apply
- Valid driver’s license and reliable transportation (if client transport is required)
