We are currently seeking a Nurse Practitioner (NP) to join our team on a contract basis (minimum 3 months). This role offers remote flexibility, with the expectation that all candidates be licensed and credentialed in New York City to serve our patient population effectively.



The Role

As a Practice Provider at POD Health, youll deliver comprehensive medical care to patients through a combination of telehealth, remote monitoring, and occasional field visits. Youll work collaboratively with physicians, Care Navigators, and other interdisciplinary team members to develop and manage individualized care plans, especially for patients managing chronic conditions. Your clinical insight and compassionate care will be key to improving patient outcomes and enhancing access to essential services.



Key Responsibilities

  • Conduct detailed patient assessments, including review of medical histories, lab work, and diagnostic results
  • Diagnose and treat acute and chronic conditions, creating evidence-based care plans
  • Prescribe medications, adjust dosages as needed, and manage prescription renewals
  • Provide preventive care including screenings, lifestyle counseling, and health education
  • Deliver care using a telehealth-first model, including remote check-ins and follow-ups
  • Collaborate closely with interdisciplinary teams to coordinate care, referrals, and follow-up
  • Support our Community Health Initiatives by helping address healthcare disparities and improving outreach
  • Ensure all documentation meets HIPAA and state compliance standards, using EMR systems like AdvancedMD
  • Monitor patient progress through Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) tools



What Were Looking For

  • Graduate of an accredited NP program
  • Active license and credentialing in New York City (required)
  • 25 years of clinical experience, preferably in primary care or telehealth
  • Bilingual in Spanish or Mandarin is preferred but not required
  • Strong understanding of HIPAA, state healthcare regulations, and virtual care standards
  • Tech-savvy with experience using EMRs (especially AdvancedMD) and virtual care tools
  • Passion for accessible care and health equity
