Key Responsibilities
<ul>
<li>Provide skilled nursing care under RN supervision in a residential or assisted‐living/home‐care setting</li>
<li>Administer medications, perform treatments, monitor patient status, and report changes appropriately</li>
<li>Implement care plans, maintain accurate documentation, and deliver patient/family education</li>
<li>Collaborate with other health professionals (RNs, aides, therapists, social workers, dietitians) for coordinated care</li>
</ul>
Requirements
<ul>
<li>Current NYS LPN license (mandatory)</li>
<li>Preferred: CHHA (Certified Home Health Aide) experience and/or home care background</li>
<li>Strong verbal and written communication skills</li>
</ul>
